About this policy
This Privacy Policy explains how BoltMed Canada handles personal information when you use our Canadian website, complete our questionnaire or request a consultation. For privacy questions and requests, email boltmedcanada@gmail.com. Please do not send medical records, identification documents or payment-card details to this inbox.
Information you provide
We collect the information you enter into our questionnaire to assess and respond to your request. This includes your province, age, height, weight, health history, medications, allergies and contact details. We also receive information you choose to include when you contact us. You can choose not to continue; without information needed to assess your request, we may be unable to provide the requested service.
How we use your information
We use your information to check service availability, understand your request, communicate with you and support assessment by healthcare professionals involved in your care. The questionnaire calculates measurements and uses your answers to determine the questions and next steps shown. Completing it does not approve treatment or create a prescription. Treatment decisions require a healthcare professional’s assessment.
Consent and your choices
We ask for your express consent before collecting health answers in the questionnaire. This consent is for your care request. It does not sign you up for marketing, authorize a payment or start a subscription. You can ask to withdraw consent by contacting us. We will explain how withdrawal affects your request and any information we must retain for legal or professional obligations.
Service providers and sharing
We use Webflow for our website and Embeddables for our questionnaire. Authorized team members and healthcare professionals involved in assessing your request may need access to relevant information. Service providers supporting our website, communications and payment functions process information for those purposes. We use Gmail to receive and respond to privacy enquiries. We may also disclose information where required or permitted by applicable law. We do not sell your health information.
Payment-card information
If you choose to save a payment card, the payment service provider processes the card information. Saving your card does not charge you or start a subscription. Permission to save a card is separate from consent to use your health information. Any later purchase requires the applicable price and payment terms to be presented and agreed.
Website analytics and cookies
We use PostHog to understand website usage and how visitors progress through the website. Our analytics policy excludes questionnaire health answers, contact details, medication choices and safety-screening results. We do not use questionnaire or checkout session recordings. Cookies and similar technologies may support website functionality and analytics. You can manage cookies through your browser settings; blocking them may affect website functions.
Retention
Our retention policy is to delete incomplete intake records after 30 days and requests that do not become clinical care records within 90 days after the test closes. Analytics retention depends on the service configuration and applicable obligations; you can contact us for details or request deletion. Records needed for ongoing care, consent evidence, payment administration, complaints or legal and professional obligations may require longer retention. You can ask about the retention that applies to your information. When information is no longer needed, it should be securely deleted or irreversibly anonymized.
Storage and protection
We limit access to personal information to people who need it for their work and require appropriate safeguards for handling it. Our service uses third-party systems; do not assume that all information is stored or accessed only in Canada. You can contact us for information about how your data is handled. No online system can eliminate all privacy risks. Unauthorized access to health or contact information could expose sensitive details about you.
Access, corrections and complaints
You can contact us to request access to your personal information, correct inaccurate information, request deletion or withdraw consent. We may need to verify your identity before acting on a request. Some requests may be subject to legal or professional restrictions; we will explain applicable limits.
